Etiology

  • Reaction to gluten

Risk Factors

  • Female > male
  • Age onset childhood to adult
  • Travels w/autoimmune dz→ DM1, thyroid d/o, Sjogrens, IgA nephropathy, other

Symptoms

  • Chronic abdominal bloating/distention, gas, diarrhea (GI symptoms predominate)
  • Wt loss/inability to gain weight
  • Fatigue from anemia (20-30%)
  • Joint pain (20-30%
  • Easy fractures from osteoporosis(10-50%)
  • Pruritic papulo-vessicular skin eruption→ dermatitis herpetiformis (4-20%)
  • Painful neuropathy (up to 30%)

Physical Exam Findings

  • Low BMI
  • Papulo-vessicular eruption on arms (predilection for extensor surfaces), scalp, knees, back, buttocks (dermatitis herpetiformis)
  • Impaired sensation and pain from peripheral neuropathy
