- Related to trauma, raynauds, severe illness, pemphigus, chemotherapy or any process severe enough to cause disruption in nail growth
- Appears symmetrically in multiple/all nails as typically all affected by same systemic process
- Grows out w/nail over time
- Horizontal disruptions/depressions/grooves in nail plate
