- Injury to the nail matrix
- Associated with renal failure, arsenic poisoning, heavy metal poisoning, chemotherapy
- Lines move distally over time as nail grows out
- Multiple white horizontal lines
- Can be one or multiple nails
- Can extend across entire width of nail
- Lines dont diminish w/direct pressure to nail plate
