- Known cancer with predilection for mets to spine→ prostate, lung, breast
- If not known previously to have cancer, then assess for symptoms suggestive of primary somewhere
- Progressive, severe, unremitting pain
- Sx related to primary
- Sometimes pain on palpation, neuro deficits based on structures affected
