CMV

Etiology

  • Virus

Risk Factors

  • HIV+ for retinitis, colitis, disseminated disease
  • S/P transplant (1-4m) for systemic dz
  • Immune modulating therapy, chemo for CMV colitis

Symptoms

  • Normal hosts get mono, just like EBV: incubation 3-8 w, then f, c, malaise, myalgias, arthralgias; sometimes RUQ discomfort; fatigue can last x 4-6+w after acute infection; assoc w/Guillain-Barre, mononeuritis multiplex and peripheral neuropathy
  • HIV +: retinitis, colitis, disseminated dz
  • Systemic infection in patients 1-4m s/p transplant
  • Compromised by immune modulating meds, chemo: diarrhea (watery or bloody) w/CMV colitis

Physical Exam Findings

  • Hepatomegaly
  • Splenomegaly
  • Head/neck adenopathy & pharyngitis relatively uncommon (c/w EBV)
