- Healthy hosts not affected
- In HIV + causes brain infection cd4 < 200 (in particular, < 100)
- Pregnant women can pass in utero→ congenital abnormalities
- Focal neuro deficits
- Eye pain and visual loss from retinal involvement
- Ring enhancing lesions (and CD4< 200) on CT suggest lymphoma, Toxo, tuberculoma (developing countries); w/Multiple lesions favors Toxo
- MRI can be very helpful, though harder to obtain acutely
- Serum Toxo IgG helpful if negative (Toxo much less likely)
- LP (if safe): Toxo PCR (specific, not sensitive)
