- Neuro/Muscular d/o→ ALS, NMJ dz, cancer
- Worse lying down
- If bilateral involvement→ worse sx than unilateral
- Abdomen may move inward (paradoxically) w/inspiration
- Absence of diaphragmatic excursion on percussion
- Decreased breath sounds at base of affected lung
