- Assoc w/immunocompromised
- CD4 < 50 or untreated HIV
- CNS disease→ sub-acute headache, fever, confusion, progressing over hours to days
- Associated with increased ICP
- Pulmonary→ subacute cough, sob, chest pain, fever
- Crackles on lung exam
- CN 6 or other CN palseys
- Weakness, numbness or other focal neuro findings
- CT or MRI helpful to r/o mass lesion (not typically present w/crytpo), assess for hydrocephalus (a potential complication)
- Serum cryptococcal antigen (CrAg), high concordance w/CSF CrAg
- LP (if safe): CSF CrAg, India Ink
