Etiology

  • Inter-ventricular connection
  • Most common congenital defect in kids
  • Can manifest in adulthood, if smaller w/pathophysiology developing over time, though this is unusual
  • Typically L to R shunting
  • Rare complication of MI (5-7d post, acute SOB and decompensation)

Symptoms

  • Endocarditis
  • Slowly progressive DOE
  • Paradoxical emboli

Physical Exam Findings

  • Systolic murmur along lower sternal border
  • Sometimes murmur of AI
  • Other findings CHF
  • Findings of endocarditis if suggestive sx
