Etiology

  • Clostridium Tetani (GPR)
  • Exposure via contaminated wounds
  • Toxin taken up by pre-synaptic side of NMJ--> travels to spinal cord--> blocks release of inhibitory neurotransmitters--> spasticity
  • Vaccination markedly decreases incidence dz w/50 cases/y US

Risk Factors

  • Puncture wounds
  • Lack of vaccination
  • Dirty, contaminated wound, often w/devitalized tissue
  • Injection drug use with contaminated needle/product, through dirty skin

Symptoms

  • ~1w incubation
  • Dysphagia
  • Drooling
  • Sweating
  • Increased tone in jaw muscles
  • Diffuse musle pain/spasms
  • Airway compromise

Physical Exam Findings

  • Fever
  • Tachycardia
  • Elevated bp
  • Lock jaw
  • Severe muscle spasm
