Contribution from primary bleeding disorder or meds that affect coaguation (heparin, warfarin, asa, etc)
Bleeding from GI source → mouth, esophagus, stomach w/aspirated blood coughed up and/or vomiting mistaken as hemoptysis
Severe HF - assoc w/very elevated LVEDP, eg w/mitral stenosis
